We appreciate your selection of our office for your eye care.

First visits usually take approximately 1½ hours. Dilation of your eyes may be required for the examination. The effects of this procedure can take several hours to wear off. If you are not comfortable driving with your eyes dilated, it is best that you arrange for other means of transportation to return home.

Please download and complete the following two forms, then return them to us prior to your appointment by FAX (408) 358-8999, using this Fax Cover Sheet, or by US Mail.

This will speed-up the new patient registration process.

Also, please bring the following items with you to your appointment:

  • A list of all your medications
  • A list of all your eye drops
  • Your eyeglasses
  • Your contact lens boxes
  • All your insurance cards
  • Copies of previous eye records
  • Visual field tests
  • Retinal and optic nerve images
  • Exam notes

As a courtesy to our other patients, we ask that if you are unable to keep your appointment, please call to reschedule at least 24 hours before your scheduled appointment time. Otherwise, you may be charged $125.00 for a missed appointment.
